Home Nurse and Physiotherapist

Overview: A Home Nurse cum Physiotherapist provides nursing care with physical therapy, delivering holistic healthcare at the patients home. This role involves assessing patients, developing treatment plans, providing nursing care, administering therapies, educating patients and families, and collaborating with other healthcare professionals.

Responsibilities:

Assessment and Care Planning: Conduct initial assessments, develop individualized care plans, and monitor patient progress.

Nursing Care: Provide skilled nursing care, including wound care, medication administration, vital sign monitoring, and personal hygiene assistance.

Physical Therapy: Implement physical therapy plans, utilizing exercises, massage, and other modalities to improve mobility, strength, and function.

Patient and Family Education: Educate patients and families about their conditions, treatment plans, and home safety measures.

Documentation: Maintain accurate and detailed patient records, documenting assessments, treatments, and progress.

Collaboration: Work closely with physicians, other therapists, and support staff to coordinate care and achieve patient goals.

Communication: Communicate effectively with patients, families, and other healthcare professionals, addressing questions and concerns.

Equipment Management: Manage and oversee the use of therapy equipment, ensuring it is safe and effective.

Patient Support: Provide emotional support and encouragement to patients and families.

Qualifications:

• Registered Nurse (RN) license.
• Bachelor of Science in Nursing (BSN).
• Masters degree in physical therapy (M.S. in PT).
• 3+ years of experience in home health and physiotherapy sessions.
• Proficiency in electronic medical record systems.
• Strong communication, interpersonal, and problem-solving skills.
• Ability to work independently and as part of a team.
• Valid drivers license and vehicle for travel to patient homes.
• Tolerance and patience
• Proficiency in English, Hindi/Urdu and manageable Arabic.
